Applying for vaccinations from abroad
How to complete vaccination series?
Have you got your 1st or 2nd dose abroad and you want to get fully vaccinated? Go to https://korona.gov.sk/en/, download Information Chart and Good Faith Declaration, fill in the correct data and sign the both forms.
Submit the both forms and the proof of previous vaccination (original and copy) to the doctor at the Vaccination Centre where you get vaccinated without prior registration.
Only the dose given in Slovakia will be shown in yout EU DCC.
COVID-19-PASS will be needed to obtain your EU Digital Covid Certificate. Go to https://korona.gov.sk/en/ and click the button „Nemám/Nepamätám si COVID-19-PASS“ which will be automatically generated and sent to your e-mail. After obtaining COVID-19-PASS you can apply for EU DCC.
